Manawanui Whai Ora Kaitiaki

The purpose of the Manawanui Whai Ora Kaitiaki(MWOK) programme is to walk alongside and empower individuals, and their whanau, who have Long term Health conditions (LTC) to successfully self-manage their conditions.

Year of Care: Care planning in long term conditions

Year of Care training is the only national training programme that commissioners can be sure will deliver the key learning and positive impact from the Year of Care pilot programme.

Enhanced primary care programme

CM Health is in the pilot phase of our Enhancing Primary Care programme. Working collaboratively with our primary health organisations (PHOs) and nine general practices, the programme creates a more sustainable model, which will release capacity in general practice teams to support more planned proactive care for our patients with complex health needs.

Personalised care planning programme

Personalised care planning aims to ensure that people with long term conditions are more involved in decisions about their care through a care planning discussion. This focuses on what is important to them with goal setting and the provision of information and support for self care. Care planning aims to address wider, holistic needs which should open up wider choices including services to support self care or services to meet other needs such as social or psychological.

The KARE project

The purpose of the KARE Project is to develop and evaluate a comprehensive package of interventions to help keep older adults healthier so they can stay at home longer.

The Flinders Programme
